Welcome to the website of Natural Touch Chiropractic and Wellness, PLLC. By accessing our website, using our online tools, or utilizing our services, you agree to comply with and be bound by the following terms and conditions. Please read them carefully.

1. Age and Consent Requirements

Users of this website, as well as anyone booking or requesting appointments through our integrated ChiroTouch systems, must be at least 18 years old. If you are scheduling an appointment for a patient who does not have their own legal medical consent (such as a minor child or a disabled person), you represent and warrant that you are the parent or legal guardian who legally holds control of the medical consent on that patient's behalf. All patients who are 18 years or older and have their own legal medical consent, must fill out their own forms and request their own appointments. 

2. Patient Messaging Consent & SMS Terms

By providing your phone number to Natural Touch Chiropractic and Wellness, PLLC, you explicitly consent to receive text messages, emails, and phone calls from us.

• Program Description: Messages may include appointment reminders, clinical updates, holiday closures, wellness tips, and clinic announcements. Messages may be sent via ChiroTouch or through alternative communication systems managed by the clinic.

• Message Frequency: Message frequency varies based on your appointment schedule, approximately 2–4 messages per appointment.

• Opt-Out Instructions: You can cancel the SMS service at any time. Just text STOP to our number. After you send the SMS message "STOP" to us, we will send you an SMS message to confirm that you have been unsubscribed. After this, you will no longer receive SMS messages from us. If you want to join again, just sign up as you did the first time.

• Help Instructions: If you are experiencing issues with the messaging program you can reply with the keyword HELP for more assistance, or you can get help directly by calling us at (123) 456-7890 or emailing drshelby@naturaltouchchiropractic.com.

• Cost & Disclaimers: Message and data rates may apply for any messages sent to you from us and to us from you. If you have any questions about your text plan or data plan, it is best to contact your wireless provider. Carriers are not liable for delayed or undelivered messages.

3. Missed Appointment Policy (AKA. No-Show, Cancellation)

We value your time and the time of our provider. A 24-hour notice is strictly required to cancel or reschedule an appointment. Failure to provide 24-hour notice, or missing a scheduled appointment, will result in a missed appointment/cancellation fee of $35 per occurrence, per patient.

4. Late Payment Policy

If any card saved on-file declines or expires, the patient will incur a $35 late payment fee, and a new, valid card on-file will be immediately required. 

5. Financial Policy, Payments, & Insurance Disclaimer

Natural Touch Chiropractic and Wellness, PLLC operates as a non-participating (non-par) Medicare provider.

• Upfront Payments: All payments for services rendered, regardless of your insurance coverage or carrier, are due upfront at the time of service. Displaying insurance information on our website or within our portal does not guarantee coverage or reimbursement.

• Card On-File Requirement: All patients are required to keep a valid credit or debit card securely saved on-file in our encrypted system. This card will be automatically charged for services, balances, equipment, exams, retail products, and applicable missed appointment fees and late payment fees. 

• HSA/FSA Card Restrictions: You cannot use a Health Savings Account (HSA) or Flexible Spending Account (FSA) card as your designated card on-file. However, you are permitted to use HSA or FSA cards in-person at the clinic using our physical card terminal at the time of service for eligible healthcare expenses.

6. Secure Third-Party Portals & Technology

Our website utilizes links to securely connect you with ChiroTouch, our Electronic Health Records (EHR) partner, for booking, patient portals, and intake documentation. While we maintain rigid data security standards and a Business Associate Agreement (BAA) with our software partners, Natural Touch Chiropractic and Wellness, PLLC is not liable for technical browser errors or issues occurring on external secure portals.

7. Governing Law

These Terms and Conditions are governed by and construed in accordance with the laws of the State of Minnesota. Any legal actions or disputes arising from the use of our services or website must be handled within the jurisdiction of Minnesota.
